STEP 1: DOWNLOAD & PRINT FORMSIf you’re applying as an individual entity, download forms HERE.

If you’re applying as an individual entity, make sure to print:

  • 5 copies of Verified Application Form
  • 2 copies of List of Vehicles and Drivers
  • 2 copies of Operator Data Sheet with 2×2 ID Photo
  • 2 copies of Statement of Financial Capability
  • 2 copies of Affidavit
  • 2 copies of Special Power of Attourney

If you’re applying as a partnership/corporation, download forms HERE.

If you’re applying as a partnership/corporation, make sure to print:

  • 5 copies of Verified Application Form
  • 2 copies of List of Vehicles and Drivers
  • 2 copies of Operator Data Sheet with 2×2 ID Photo
  • 2 copies of Statement of Financial Capability
  • 2 copies of Affidavit
  • 2 copies of Board Resolution
  • 2 copies of Secretary’s Certificate

STEP 2: FILL UP & SIGN FORMS

Fill up all the fields and sign where necessary. You may see sample completed and signed applications HERE.

IMPORTANT: Make sure that signatures are all original. Scanned signatures are not allowed.

STEP 3: NOTARIZE DOCUMENTS

If you’re applying as an individual entity, make sure to have the ff documents notarized:

  • 5 Verified Application Form
  • 2 Operator Data Sheet with 2×2 ID Photo
  • 2 Statement of Financial Capability
  • 2 Affidavit
  • 2 Special Power of Attorney

If you’re applying as a partnership/corporation, make sure to have the ff documents notarized:

  • 5 Verified Application Form
  • 2 Operator Data Sheet with 2×2 ID Photo
  • 2 Statement of Financial Capability
  • 2 Affidavit
  • 2 Board Resolution
  • 2 Secretary’s Certificate

 

STEP 4: SUBMIT FORMS & OTHER DOCUMENTS

If you’re applying as an individual entity, make sure to compile:

  • 5 notarized Verified Application Form
  • 2 List of Vehicles and Drivers
  • 2 notarized Operator Data Sheet with 2×2 ID Photo
  • 2 notarized Statement of Financial Capability
  • 2 notarized Affidavit
  • 2 notarized Special Power of Attourney
  • 2 photocopies of  of Proof of Filipino Citizenship (either valid Philippine Passport or authenticated birth certificate from NSO issued within 6 months prior to submission of application)
  • 2 photocopies of Income Tax Return (BIR Form 1701) or BIR Certificate of Registration (BIR Form 2303)
  • 2 photocopies of Proof of Financial Capability (Bank Statement since January 2016 to current date or Bank Certificate with P 10,000 balance)
  • 2 photocopies of Certificate of Business Name issued by DTI (choose Regional)
  • 2 copies Map of Garage (you may use Google Maps to take a screenshot and print)
  • 2 Photocopies of all your drivers’ Professional Driver’s Licenses
  • 2 Photocopies of all your drivers’ NBI Clearance
  • 2 Photocopies of all your drivers’ Police Clearance
  • 2 Photocopies of all your vehicles’ Official Receipt and Car Registration (or alternatively, Sales Invoice and Delivery Receipt)

If you’re applying as a partnership/corporation, make sure to print:

  • 5 notarized Verified Application Form
  • 2 List of Vehicles and Drivers
  • 2 notarized Operator Data Sheet with 2×2 ID Photo
  • 2 notarized Statement of Financial Capability
  • 2 notarized Affidavit
  • 2 notarized Board Resolution
  • 2 notarized Secretary’s Certificate
  • 1 photocopy of Articles of Partnership/Incorporation and By Laws
  • 1 photocopy of Certificate of Good Standing from SEC (only if incorporated for more than 2 years)
  • 2 copies Map of Garage (you may use Google Maps to take a screenshot and print)
  • 2 Photocopies of all your drivers’ Professional Driver’s Licenses
  • 2 Photocopies of all your drivers’ NBI Clearance
  • 2 Photocopies of all your drivers’ Police Clearance
  • 2 Photocopies of all your vehicles’ Official Receipt and Car Registration

STEP 5: CLAIM PA AND CPC

GrabCar Team will contact you via mobile or email to have your Provisional Authority (PA) and/or Certificate of Public Convenience (CPC) picked up at the Grab office. Only the ff will be allowed to pick up your PA and/or CPCP:

  • TNVS Applicant/Vehicle Owner
  • Authorized representative (IMPORTANT: Please bring authorization letter with photocopy of valid government IDs of both TNVS Applicant and authorized representative)

Kindly bring full payment upon claiming of your PA. The breakdown of the P5,600 is as follows:

Filing fee for new CPC 510.00
Legal research fee 10.00
Filing fee for Provisional Authority 240.00
Legal research fee 10.00
Passenger Accident Insurance 1300.00
Publication fees 500.00
Other legal fees 2000.00
Other admin fees 1030.00
TOTAL 5600.00
